Summary
  • The purpose of this presentation is to describe and demonstrate the technique for a venous cutdown. This objecive is achieved with the aid of drawings and diagrams in addition to the step-by-step demonstration of the technique on an elderly male clinical patient
  • In this presentation the necessary equipment is assembled including the intravenous fluid, armboard, extra light, sterile dressings, prep tray, gloves, catheter and cutdown tray. Anatomical sites suitable for venous cutdown are discussed. The basilic vein is selected for this demonstration
  • The tourniquet is applied and the skin is cleansed with alcohol and painted with a nonirritating iodine solution. Drapes are placed and the skin over the area is infiltrated with a local anesthetic agent. The skin is incised deeply enough to expose the fatty layer below the skin. A hemostat is used to spread the edge of the incision. The vein is located and isolated using two pieces of suture. The distal suture is tied and the venous catheter is trimmed to provide a rounded bevel. It is filled with saline solution
  • Holding the vein taut a small pair of scissors is used to cut into the vein. The catheter is passed into the vein and the tourniquet is released. The catheter is advanced as far into the vein as necessary and tied in place with the proximal suture. The distal suture is tied around the vein to prevent slippage. The incision is closed with interrupted sutures. An antibiotic ointment and dry sterile dressing are applied and the catheter is attached to the intravenous line
